GOFER ORDER FORM

Although the texts of the Thesaurus Musicarum Latinarum (TML) can be
searched by any of a number of programs, the TML has recommended and
continues to recommend "GOfer," a powerful software utility for searching,
finding, and retrieving text. The program, which runs in either MS-DOS or
Macintosh environments, is produced and owned by Microlytics, Inc., Two
Tobey Village Office Park, Pittsford, N.Y. 14534. Microlytics no longer sells
GOfer, but as a service to its subscribers, the TML has obtained a non-exclusive
license to distribute the product for a minimal charge. If you would like to
obtain a copy of the program, please retrieve, print out, and complete the
following order form and return it to us by conventional mail as noted below.

NOTE: THE THESAURUS MUSICARUM LATINARUM (TML) ACTS IN
THIS TRANSACTION SOLELY AS A SELLING AGENT AND ASSUMES
NO RESPONSIBILITY FOR THE PERFORMANCE OR SUITABILITY OF
THE PRODUCT. IF THE COPY OF THE PROGRAM SUPPLIED BY THE
TML IS DEFECTIVE, IT WILL BE REPLACED WITH A NEW COPY, BUT
NO REFUNDS CAN BE GIVEN. THE TML SHALL NOT BE RESPONSIBLE
FOR ANY DIRECT, INDIRECT, CONSEQUENTIAL, OR INCIDENTAL
DAMAGES ARISING OUT OF THE USE, THE RESULT OF USE, OR
INABILITY TO USE THIS PRODUCT. ALL OTHER WARRANTIES,
EXPRESSED OR IMPLIED, ARE ENTIRELY THOSE OF THE
MANUFACTURER.
